Report Your Claim Online
To report your claim online, simply fill out the form below and provide the necessary documents:
- A medical report stating your medical case.
- Detailed medical records, including X-ray and lab results as prescribed by your doctor.
- Invoices for all expenses you wish to claim.
- Additional documents that may be requested.
We Follow Up with You Right Away
Once you have completed the medical claim report, we will send you a prompt confirmation email. You will then be able to view and manage your claim right away.
A JICO agent will call you to explain the next steps and confirm your details. This same agent will also be able to answer any questions you may have. If you submit your claim later than 2:00 PM, we will usually call you the following morning.
Report Your Claim Over the Phone
Our Customer Care agents are available from 8:00 AM to 8:30 PM all week long. Give them a call and they will promptly handle your request. Make sure to have the following information at hand, including:
- A medical report stating your medical case.
